Celebration Athletic Club Liability Waiver

**In consideration of being allowed to participate in any way in the programs, related events, and activities of Celebration Athletic Club, the undersigned acknowledges, appreciates, and agrees to the following:**

1. **Acknowledgment of Risk**: I recognize and understand that martial arts and related activities involve the risk of serious injury, including permanent disability, paralysis, and death, which may be caused by my own actions, or inactions, those of others participating in the event, the conditions in which the event takes place, or the negligence of the "releases" named below. I fully understand that there may be other risks either not known to me or not readily foreseeable at this time.

2. **Assumption of Risk**: I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releases or others, and assume full responsibility for my participation.

3. **Compliance with Terms and Conditions**: I agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately.

4. **Release and Hold Harmless**: I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release and hold harmless Celebration Martial Arts Academy, its officers, instructors, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct the event ("Releases"), with respect to any and all injury, disability, death, or loss or damage to person or property, whether arising from the negligence of the releases or otherwise, to the fullest extent permitted by law.

5. **Medical Treatment**: I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during this activity.

6. **Media Release**: I grant Celebration Athletic Club the right to take photographs and video footage of me/my child and to use these materials for promotional and marketing purposes without compensation.

7. **Severability**: If any provision of this agreement is found to be unenforceable or invalid, the remaining provisions shall be fully enforceable and remain in effect to the fullest extent permitted by law.

**I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.**
